This form is for students, caregivers, and staff to use to refer a student who is experiencing academic, socio-emotional, or mental health challenges that are impairing a student's academic success.

STAFF: Please reach out for a consultation with the assigned counselor, school social worker or Restorative Justice facilitator for Tier I interventions before making a Tier II/III referral.
